I think it's official now, i hear this like 3 times a week.
idle air control valve (IACV)
fast idle thermo valve (FITV)
there is a valve on the bottom of the throttle body, it has 2 coolant hoses coming out of it, and is attached with 3 10mm bolts. remove it, and take out the 2 8mm bolts on the flat side. then screw down the giant plastic screw.
otherwise, check vacuum lines, and the idle screw.

This post comes with a guarantee: I guarantee that I don't care what you think about it.

you can check the iacv and the fit with out removing them...
pull your intake off the throttle body, with the car running you need to put your finger over the lil hole on the inside top of the throttle body, if it stops surging, you need to clean your iacv with some brake cleaner or make sure you has a tight fit against the I/M...
if it continues, put your finger over the lil hole on the inside bottom of the throttle body, it it stops, you need to tighten the thermo the way spork described...
i had a problem myself a few months ago, turned out my pvc hose came off and was causing my problem...

does it die/bog at all when you try and drive it? if it does it your IACV. if it fluctuates (surges), its your FITV. Have you checked your throttle cable at all? its possible that it may be a little sticky.
let us know what you find out. and good luck!

did you clean the iacv with brake cleaner and were you thourgh, it helps make a good seal to the intake manni if you clean the manni side as well, just spray some brake clean on a rag and wipe the dirt and grim off, then take some motor oil and put a lil bit on your finger, spread it on the rubber on the mounting side of the iacv, you just need a tiny bit, then bolt it up...
as far as the rattles go... im not sure about that one...
